2. 最高學歷之外國學校畢業証書影印本，並附該證書經公證之中文或英文翻譯本及該學程之全部英文成績英譯本 One photocopy of the applicant’s graduation certificate or diploma from the highest foreign educational institution completed, in notarized Chinese or English translation. One copy of the applicant’s transcript in English.

4. 健康證明書(包括人類免疫缺乏病毒檢驗報告、胸部X光檢查報告、腸內寄生蟲糞便檢查報告、梅毒血清檢查報告、麻疹及德國麻疹抗體陽性報告或預防接種證明、漢生病) A health certificate (including HIV test results, chest X-ray, stool examination for parasites, serological test for syphilis, measles and German measles antibody or proof of inoculation, and Hansen’s disease).
